Ransomware, a failed disk, a "delete" that wasn't meant for that folder, a server that shuts down and never comes back. It's not a question of "if" but "when":

We define what gets backed up, how often, and where, following the 3-2-1 standard: at least three copies, on two different media, with one copy off-site — with RPO/RTO defined by each system's criticality. Without this, "having backup" doesn't mean much.
We configure immutable repositories (hardened repository / object lock): not even an attacker with administrator credentials can delete or encrypt the copies during the retention period, with encryption of data in transit and at rest.
A backup that was never restored is a risk disguised as peace of mind. We run periodic restore tests (sandbox / automatic verification) to confirm that, when the time comes, you'll be able to recover — with a success/failure report for every job.
If a backup job fails at 2am, we don't find out at month's end — we act the same day, with a monthly backup compliance report.
Physical or virtualized (VMware, Hyper-V): your critical information needs a serious backup.
The immutable copy is, today, the last real line of defense when everything else fails.
